Total hip arthroplasty for severe hip flexion deformity in patients with ankylosing spondylitis. Clinical analysis of total hip arthroplasty for severe hip deformity in ankylosing spondylitis. Methods Twenty-eight patients (40 hips) with ankylosing spondylitis who underwent total hip arthroplasty were male. They were 21-48 years old with an average of 34.68 years. The course of disease was 3 to 16 years, with an average of 8.53 years. The buckling deformity angle is 30°-90° with an average of 60.13°. There were 16 cases on one side, 12 cases on both sides, and 16 cases (24 hips) with hip ankylosis. A modified anterior and lateral joint incision of the hip joint was used, 21 cases (29 hips) of bioprosthesis and 7 cases (11 hips) of mixed prosthesis.
Preoperative and postoperative hip function were evaluated using Harris score. The femoral prosthesis was performed using Gruen's 14-segment method, and the acetabular prosthesis was imaged using the DeLee and Charnley methods. Results The follow-up time was 12, '-64 months, with an average of 42 months. At the last follow-up, the total joint activity was an average of 29.88 from preoperative. (0°-95°) increased to an average of 124.75° (90° to 175°) after surgery. The degree of flexion deformity decreased from an average of 60.13° (30°-90°) before surgery to an average of 6° (0° to 10°) after surgery. The Harris score increased from an average of 32.88 points (12-58 points) to an average of 88.23 points (69-98 points), an excellent 20 hips (50%), and a good 15 hips (37.5%). It can be 3 hips (7.5%), 2 hips (5%), and the excellent and good rate is 87.5%. X-ray examination showed no signs of loosening of the prosthesis except for one case of femoral perforation fractures and one case of early hip dislocation. Heterotopic ossification occurred in 8 hips (20%). There was no significant difference in Harris score between bioprosthesis and mixed prosthesis (P>0.05).
